Instructions
All of the information contained in this survey is strictly confidential and no identifying characteristics will be used in the summary report. Please fill in the hourly wage for the categories below that pertain to your facility. If a department manager is managing more than one department, please enter their wage rate in both departments, i.e. if laundry and housekeeping are being managed by one individual, please enter their wage rate in both locations. Please note that all wage related answers should be hourly and in numbers only, i.e. 12.50. If you do not have an answer for a specific question, enter a zero.
