Annually, KofC Supreme requests all councils provide the hours our council's brothers supported council, church, and community programs.

To make this survey quicker for you, we’ve already captured your hours spent supporting KofC-sponsored events in a separate document. However, we still need to know your hours spent for other volunteer projects, sick/bereaved visits and hours, and blood donor visits from 1 Jan 2017 to 31 Dec 2017.
 
This year, at the end of the survey, we added an optional area for you to let us know if you'd like to make your professional services or personal skills available to our council brothers. The information will only be available from our council website.
 
 

* 1. Sick. Number of visits you made to the sick/nursing homes/hospitals.

* 2. Bereaved. Number of condolence visits.

Note: Also include number of attended funerals and wakes.

* 3. Blood Donor. Number of visits you served as a blood donor.

* 4. Community Volunteer Service. Number of hours for:

Church-related hours, such as PSR staff/teacher, Greeter, Legion of Mary Ministry, Prison Ministry, CRHP Ministry, RCIA Program staff/teacher, Parish Council, Coffee and Doughnuts Ministry, Funeral Ministry, etc.

Note: Please don't count your hours with: Keep Christ in Christmas, Mass of Remembrance, and the Acolyte Program. We already counted your hours with these council activities.

* 5. Habitat for Humanity hours. Note: Please count your hours with any Habitat for Humanity weekend except for our KofC 2017 work day. We already counted your hours for that day.

* 6. Food for Families hours. Consider any hours with the Saint Vincent de Paul Food Pantry, local food bank, etc.

Note: We already counted your hours with our council's Feed the Needy Program.

* 7. Community-related hours. Examples are Saint Vincent de Paul case work/projects, Homeless Shelter, Cristo Rey advisor/volunteer, Monastery volunteer, Notre Dame Academy volunteer, diocese volunteer, Centro Hispano Marista staff/teacher, People Making Progress, hospital volunteer, library volunteer, Interfaith Outreach, school volunteer, clean-up days, local community volunteer projects, etc.
 
Note: Don't count your hours with: I Care, Fish Fry, Pumpkin Patch, Desserts w Santa, cookouts, Fall Festival, Vidalia Onions, or the two 2016 KofC-sponsored Homeless Shelter nights. We already counted your hours for these activities.

* 8. Youth-related hours, such as Boy Scouts, Cub Scouts, Girl Scouts, Life Teen, CHAMPS, coaching team sports, etc.

Note: Don't count your hours with: Basketball Free Throw, Altar Server Appreciation Day, Family Movie Night, and Pre-School Cookout activities. We already counted your hours for these activities.

* 9. Culture of Life-related hours, such as Birthright, Pregnancy Aid Center, Walk for Life, etc.

Note: Don't count your hours with the parish's Baby Bottle Campaign.

* 10. Veterans Administration Volunteer Service (VAVS)- related hours.

* 11. Special Olympics-related hours.

* 12. Miscellaneous activities hours. Any remaining volunteer service hours in areas not noted above.

* 13. Number of hours for the sick/disabled members and their families. Examples are household chores, transportation, tutoring, counselling, etc.

* 14. Your contact information. Please enter your nickname.

* 15. Your contact information. Please enter your last name.

* 16. Your contact information. Please enter your preferred contact phone number if we have questions.

* 17. (Optional) Professional Service Listing. List the service you'd like your brothers to know that you can provide for a fee. Please include when you're available (on call, full-time, part-time).

* 18. (Optional) Volunteer/Non-Paid Service. List the service/skill you'd like to offer to other brothers at no cost. Examples could be basic plumbing, graphic arts, woodwork, PC skills, accounting, sailing classes, ... you get the idea.
