Vaccine Questionnaire

Thank you for helping us determine the correct amount of vaccine Kendal requires.

Walgreens needs to know the amount of vaccine to bring to Kendal. They plan to vaccinate all willing residents on Wednesday, December 30 and all willing staff on Thursday, December 31. Please help us provide an accurate number of vaccines needed.

Please do not include a household in this questionnaire. We need EACH individual to answer the survey to obtain proper numbers.
