Study on Bicycle Accessibility and Safety Among Kentucky's Youth |
Survey Consent
Dear Participant:
You are being invited to participate in a research study about youth access to and safety using bicycles across the state of Kentucky. National, state, regional and local constituents with interests in the health, wellness and safety of youth related to their interest in, accessibility to and safety practices with bicycling are consistently working to gather data to support bicycling as a life-long activity. Your responses to the survey will gather data about the accessibility and safety related to your child's bicycling.
You are being invited to participate in a research study about youth access to and safety using bicycles across the state of Kentucky. National, state, regional and local constituents with interests in the health, wellness and safety of youth related to their interest in, accessibility to and safety practices with bicycling are consistently working to gather data to support bicycling as a life-long activity. Your responses to the survey will gather data about the accessibility and safety related to your child's bicycling.
The online survey will take approximately ten minutes to complete. You can skip any questions you do not wish to answer. The risks associated with this project are minimal and there are no direct benefits to you.
Your name and other identifying information will not be collected. Your responses will be anonymous. “Your completed survey data in raw downloadable form will be stored electronically at Midway University on a password protected device for a period of 3 years following completion of the study, at which time all data will be destroyed by deletion”. Individuals from the Sport Management and Marketing Programs and the Midway University Institutional Review Board (IRB) may inspect these records. In all other respects, however, the data will be held in confidence to the extent permitted by law. Should the data be published, your identity will not be disclosed.
Your participation in this study is completely voluntary. By completing the electronic survey, you are voluntarily agreeing to participate and share information about your child's bicycling access and safety.
You acknowledge that all your questions have been answered in language you can understand. If you have any further questions about the study, please contact Dr. Jeb Gorham – jgorham@midway.edu – 859-846-6290. If you have any questions about your rights as a research participant, you may contact the chairperson of the IRB at robert.barrett@midway.edu. The IRB is an independent committee of university faculty, staff, and members of the community. It is established to protect the rights of individuals involved in research studies. You will be given the opportunity to discuss any questions about your rights as a research participant, in confidence, with a member of the committee. Midway University IRB has reviewed this study.
