Do You Have a Liquor License and Do You Intend to Provide Liquor Service in Your Proposed Temporary Expanded Service Area?  If "Yes," Then You Must Also Fill Out and Submit the State of Colorado "Permit Application and Report of Changes" Form as Required by the Liquor Enforcement Division and the City of Westminster. This Form and Instructions for Submittal are Available Here. This Form Must be Submitted Directly to the City Clerk's Office for State and City Approval.
