East Toronto Health Partners Home Care Client Survey

East Toronto Health Partners (ETHP), the Ontario Health Team serving East Toronto, is a group of more than 50 health and community care organizations in East Toronto working together to improve the way people find and get care. We are testing new ways to deliver and improve home care in your neighbourhood and we would like to gather information from community members. We will use this information to help us in planning and improving home care services in East Toronto.
