NGO Health & Disability Network - Join the network and influence what we do

This registration form is for health and disability not-for-profit NGOs that would like to join the NGO Health & Disability Network so they can participate in our activities and nominate or vote for representatives on the NGO Health & Disability Council that engages regularly with the Ministry of Health.

This membership form supports the voting criteria outlined on the NGO website www.ngo.health.govt.nz.

Once registered, NGOs will remain on the voting register until they opt off. Membership is completely FREE.

Each year, NGO election registrations and nominations for candidates will be closed off at 6pm on 5 October.

Question Title

1. CRITERIA
To be eligible to join the Network and register to vote, your NGO must:
- Conduct activities consistent with the promotion of health or the delivery of health and disability services in the relevant sector
- Be non-governmental and non-profit
- Be a clearly identified New Zealand organisation (which may include NZ branches of international NGOs).
- Receive Vote Health funding (ie: funding from the Ministry of Health and/or District Health Board/s) in relevant health and disability sub-sectors (see below).
