The Chronic Health Assessment Plan (CHAP) for people with intellectual disability is a health assessment tool with a section for the person with intellectual disability and their support person to fill in before taking it to the GP. Please choose a response that best describes your level of agreement with the following statements about the potential use of the CHAP, or something similar, in South Australia. (Choose all options that apply)

Strongly Agree Agree Neither agree or disagree Disagree Strongly Disagree
The CHAP tool would improve the quality of annual assessment of people with intellectual disability in South Australia.
The CHAP tool provides GPs with more information about the health of people with intellectual disability before they do their annual assessment because of the information provided by the person and their support person.
If disability organisations provided their clients with a copy of the CHAP tool and supported them to have their annual health assessment more people with intellectual disability would receive an annual health check up.
The CHAP tool is better than the current Medicare template used to assess people with intellectual disability.
A paper based CHAP tool would be helpful until a computerised option could be created.
The CHAP tool will only work if it is a computerised template.
