2025 Many Faces Award Nominations |
The Many Faces of Community Health Conference Planning Committee is in the process of preparing for the annual conference. This will be 20th anniversary of the Many Faces of Community Health Conference, which will take place in person on October 1-2, 2025 at the Heritage Center of Brooklyn Center in Brooklyn Center, MN.
The Many Faces of Community Health Awards are intended to recognize individuals and organizations that have been at the forefront of leading innovations in health care, improving care delivery models, and promoting health. We would like to pay special recognition to individuals who have championed community health by working across multiple organizations.
We are pleased to announce that the nomination period for these two annual awards is now open! We are inviting you to submit nominations for one or both of the award categories. Select an award on the following page to learn more about eligibility and selection criteria, and to submit a nomination. Nominations are due by COB on Wednesday, September 17th for the awards:

Award winners will be recognized live during the 2025 Many Faces of Community Health Conference. Winners will receive a complimentary registration to attend the conference.
