Thank you for participating in our survey! This survey is intended for professionals involved in identification, screening, referral, assessment, diagnosis, support and follow-up related to Fetal Alcohol Spectrum Disorder (FASD).
Your responses will inform the development of a HUB containing educational materials, tools and resources for health and allied health professionals and families related to FASD diagnosis. It will also provide important information about diagnostic criteria of FASD and where updates are required to diagnostic guidelines.
All responses are confidential, unidentifiable and will be reported only in aggregate. Results will be shared with FASD diagnosis clinics across Canada.
The survey takes approximately 15 minutes to complete. Your participation is voluntary.
