The Ladue Middle School Parents Association (“LMSPA”) is sponsoring an 8th Grade Year-End Party for all current Ladue Middle School (“LMS”) students on May 18, 2019 from 7:30-10:00 p.m. at LMS, with a pre-party from 6:30 p.m.-7:30 p.m. on the same date (the “Party”). All students who attend the Party are required to sign a Permission Slip and Liability Waiver as condition to entry into the Party. You may complete the Permission Slip and Liability Waiver online by completing ALL of the following 10 questions, which includes an electronic signature at the end.
