As a client or potential client of a home visiting program offered by our Central and North Central Florida Coalitions (Healthy Start, TEAM Dad, Parents as Teachers, Nurse-Family Partnership, or NewboRN Home Visiting program) your thoughts and experiences are important to us. We are conducting a community needs assessment and your input will help us better understand local needs to determine how we can improve services, health outcomes, and the well-being of individuals and families in your (and our) community.
This survey should take no more than 10 minutes to complete. It's anonymous, your responses will be kept confidential, and the information you provide will not be used to identify you. To be eligible, you must be 18 years of age or older and live within our 16-County service area (Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Levy, Marion, Putnam, Sumter, Suwannee, and Union counties).
