Thank you for taking the time to complete the Faith Community Nursing demographic survey. The purpose of this survey is to describe Faith Community Nurses (FCN) and FCN coordinators' practice. The idea for the survey started with a conversation amongst members of the Health Ministries Association (HMA) Program Leaders Constituency Group. The Westberg Institute for Faith Community Nursing (Westberg Institute), formerly the International Parish Nurse Resource Center, has received IRB approval for this survey. Experts from HMA, Westberg Institute, and Faith Community Nursing International (FCNI) participated in the editing of this survey. The survey data will be analyzed and presented at the 2016 HMA National Conference and will be published in Perspectives, HMA Today and the International Journal of Faith Community Nursing.
