This form should be utilized by students who have concerns about the learning environment. This includes behaviors and comments that have a negative effect on student learning or the faculty-student relationship. Comments in this form will be reviewed by the Office of Student Affairs and communicated to the Course Director and other members of leadership when necessary.  At no point will the identity of any student be revealed to any faculty, staff, students, or outside parties without express permission from the student.  For urgent concerns, please contact Ms. Melanie Brown directly by email or phone (504-568-4875).
