INFORMED CONSENT FORM
A Quantitative Study of Perceived Life Satisfaction and Resilience in Divorced Women

INTRODUCTION
The purposes of this form are to provide you (as a prospective research study participant) information that may affect your decision as to whether or not to participate in this research and to record the consent of those who agree to be involved in the study.

RESEARCH
Mary Katherine Bush Best Cornell, doctoral student has invited your participation in a research study.

I am completing this research as part of my doctoral degree.

STUDY PURPOSE
The purpose of the research is to study life satisfaction and resilience in divorced women.

ELIGIBILITY
You are eligible to participate in this research if you:

1. Have been divorced one time only
2. Are single
3. Are between the ages of 30 and 50
4. Make more than $36,001 a year
5. Are female
6. Are American
7. Are heterosexual

You are not eligible to participate in this research if you:

1. Are male
2. Are not an American
3. Are not heterosexual
4. Are married
5. Have been divorced more than once
6. Are less than thirty years old
7. Are more than 50 years old
8. Earn less than $36,001 a year

DESCRIPTION OF RESEARCH ACTIVITY
If you decide to participate, then as a study participant you will be asked to:

1. Read the recruitment letter for about three minutes.
2. Read and sign the informed consent letter for about three minutes.
3. fill out the demographic questionnaire for about five minutes.
3. Read and fill out the Satisfaction with Life survey for about five minutes.
4. Read and fill out the Connor-Davidson Resilience Scale for about 10 minutes.

This may take about 26 minutes total.

You may skip questions.
Approximately 106 subjects will be participating in this research study.

RISKS
If you decide to participate in this research study, then you may face some risks such as: A hard time if you do not feel satisfied with your life or if you do not feel strong. You may have strong feelings about divorce that may be hard.

To decrease the impact of these risks, you can:

Skip any item in the survey, and/or, stop participation at any time. You may also email me, or use the websites that you saw this study on to help you.



BENEFITS
If you decide to participate, direct benefits to you are: Thinking through your satisfaction with life and your resilience. You may feel grateful or find an area that you can work on with a friend, counselor, or family member.
If you decide to participate indirect benefits to you are: Seeing your own levels of life satisfaction and resilience, and helping with research in this area.

ANONYMITY
All information obtained in this study is strictly anonymous unless disclosure is required by law. The results of this research study may be used in reports, presentations, and publications, but the researcher will not identify you. In order to maintain anonymity of your records, Mary K Bush Best Cornell, the researcher, will keep your information anonymous, assign a number code for each person, and delete all the data on the computer after three years.

The people who will have access to your information are: Mary K Bush Best Cornell, my dissertation chair, and my dissertation methodologist.

I will secure your information with these steps: All data will be on my laptop in my locked home. All data will be able to be accessed through a password known only to me.

I will keep your data for 3 years. Then, I will delete electronic data and destroy any paper data.

WITHDRAWAL PRIVILEGE


It is ok for you to decline to participate in this research study. Even if you say yes now, you are free to say no later, and stop participating at any time. There will be no penalty to you.

If you decide to stop participation, you may do so by: Not filling out the surveys. You may also email me and tell me that you do not want me to use your information in this study. If so, I will not use the information I gathered from you.

Your decision will not affect your relationship with Grand Canyon University or otherwise cause a loss of benefits to which you might otherwise be entitled.

I may stop your participation, even if you did not ask me to, if: You not meet the things that are needed for this study.

COSTS AND PAYMENTS
There is no financial cost to you as a participant in this study. There is no payment for your participation.

COMPENSATION FOR ILLNESS AND INJURY
If you agree to participate in the study, then your consent does not waive any of your legal rights. However, no funds have been set aside to compensate you in the event of injury.

VOLUNTARY CONSENT
