Thank you for taking the time to complete this survey. Your feedback is vital as we reflect on board members' experience, engagement, and CB12M priorities. This survey is issued only to CB12M members and public members. Please do not share this confidential survey as it may impact its outcomes.

How will this information be used?

Your feedback will be used to shape and improve our committee structures, training and guidance documents, and implement initiatives as needed. A summary of responses will be shared with the Executive Committee and each Committee Chair to implement recommendations made and create improvements. Your name and any identifying information will be kept confidential by Elected Officers and Board Staff. While not required, sharing your name will allow us to make changes to committee assignments and reach out if you have additional questions or follow up on comments.
