Carbon County Health Department is rebuilding!
We are excited to be building a public health department that serves the needs for all of our community members- from infants through seniors.
By taking part in this survey, you will give us insight into what programs, services, and resources YOU want and need. It is critical that we understand what our communities need so that we can provide specifically tailored programs and assistance to the residents of Carbon County. Creating these tailored programs allows us to better support you, your health, and the health of the community.
Participation in this survey is entirely voluntary. It will take approximately 15-20 minutes to complete this survey. Your answers are completely anonymous and will never be linked back to your name. There are no known risks to participating in our survey. You have the right to not answer a question or completely withdraw from participating at any time. By participating in this survey, the Carbon County Health Department will be able to use your responses to help tailor health promotion efforts to help improve your health and the health of your community.
At the end of the survey, and as a thank you for participating, you will be given the option to click an external link that will take you to another webpage. Here you can provide your name and phone number and be entered into a drawing for a prize! Again, your name will not be tied to your previous responses.
We are also conducting a simultaneous survey about the needs of children and child care within Carbon County. That survey opportunity will also be linked at the end of this survey.
