Canadian restaurant operators who are Hellmann's customers are invited to sign up for the Hellmann's Hockey Mayolis activation running from October 8 to December 11. By submitting this form you confirm you are an existing Hellmann's customer. Please note you may be required to provide a proof of purchase. Submission of this form provides consent to participate and be contacted by Hellmann's and Unilever Food Solutions.

The locations are for two purposes: (1) to target guests within a 5 km radius of your restaurants, and (2) send in-restaurant collateral material. The email address will be used for general contact purposes, and to share metrics from the paid media program targeting guests near each location. 11.All participating restaurants will receive contest cards to distribute to guests who purchase a Hellmann's menu item. Contest cards are for guests to scan the QR code to enter the contest.
