Seniors Active Living Centre (SALC) Member's Survey

During each funding year, SALC operators must survey SALC members and participants to determine their level of agreement with the following statements related to the outcomes of the SALC program.
Levels of agreement are based on a five point scale:
strongly agree (5), agree (4), neither agree nor disagree (3), disagree (2), strongly disagree (1), not applicable/don’t know (0)
