Purpose
This form is used to nominate a young person who has used their voice, lived experience, and leadership to create positive change for themselves or others.
Important Information
Please discuss this nomination with the young person before submitting it to ensure they are willing to participate.
Once nominated, the nominee will receive a separate form to complete. Both the nominators form and the nominees form must be submitted for the nomination to be considered complete.
Eligibility
Nominees must have lived experience in an out-of-home placement. This may include foster care, kinship care, supervised independent living placement, certified or licensed facility, or placement through the juvenile justice system (including commitment to the custody of the state department of human services and placement in a community setting).

To ensure this award recognizes as many young leaders as possible, previous recipients of the Foster Youth Voice Celebration Award are not eligible for future nominations.
