Stockport Council is reviewing its information for parent/carers and young people on transition to adulthood.  PACTS is working with Stockport Council and would like some parent/carer views on the Transitions Guide for Parents and Carers, which was first published in 2018.

You can see this booklet on the Local Offer at

Please look at the booklet and then answer the questions in this short survey. If you have time and want to do so, you can also look at similar guides which other areas have produced.  They could give you ideas about how to make the Stockport guide better:

South Gloucestershire: https://www.sglospc.org.uk/publications

Suffolk: https://search3.openobjects.com/mediamanager/suffolk/enterprise/files/suffolk_16_transition_guide.pdf

The survey closes on Wednesday 20th January.
