Thank you for taking a moment to give us your feedback. The Road to AIDS 2012 is an effort to ensure that the voice of the community is part of the discussion to address HIV/AIDS in communities across the United States. Town hall meetings held across the country will include people living with HIV/AIDS, all levels of government, advocates, business partners, media, and others. The meetings will engage community members and Federal and local government officials in a conversation on how to best address HIV/AIDS. Your participation in this survey adds to that conversation.
This survey will take approximately 10 to 15 minutes to complete. The survey has multiple choice and open ended questions. Please be brief with your answers to open ended questions. Keep your focus on the implementation of the National HIV/AIDS strategy; how the Patient Protection and Affordable Care Act (health care reform)affects people living with HIV/AIDS and HIV/AIDS services; and looking forward to the International HIV/AIDS Conference in 2012. Please answer all questions.

Your responses are completely confidential. Your email address, IP address, and other identifiers are not being monitored by this online system.
