Introduction to the Special Medical Needs Registry

Question Title

Thank you for taking the Special Medical Needs (SMN) survey. This survey is designed to identify the locations and needs of the citizens with special medical needs. This information will be used in the event of an environmental emergency like a hurricane, tornado or ice storm.

A person with special medical needs is defined as an individual with health needs that require daily medical attention and would be adversely affected by the interruption of services such as utilities, transportation, or daily professional care.

Once the survey is complete, you will be assigned a priority level according to our Special Medical Needs policy/guidelines. Educational information will be mailed to you regarding how to shelter at home or where to go in case of an emergency.

In the event of a disaster, we will coordinate with home health and personal care agencies, home medical equipment providers, pharmacies, and transportation services to assure that essential services are available.

Our goal is to assist in the process whereby persons can remain at home in a healthy, safe environment or relocate the person to such a place on a temporary basis. A Special Medical Needs shelter will be opened once other options have been exhausted.
