Important: Please read the Network Development Learning Collaborative (NDLC) Charter carefully. It describes the purpose, expectations, and support that will be provided to learning collaborative participants.
 
The deadline for this survey is March 19, 2018.
 
Number of participants. In collaboration with the Administration for Community Living, the National Council on Aging will select up to ten organizations to participate in this eleven-month long learning collaborative. Selection will be based on review of the applications to determine which organizations can benefit the most from this type of experience. Organizations should coordinate with others in their designated coverage area or state and identify one organization as the lead on the learning collaborative initiative.
 
Eligible applicants. This learning collaborative is designed for organizations that are in the early to intermediate stage of developing their business acumen and organizational strategies.  The ultimate goal for these community-based organizations (CBOs) is the development of peer networks to create local/regional and/or statewide network hubs for the provision of LTSS services, including evidence-based programs like CDSME, Falls Prevention, etc. These network hubs will provide complementary services to be packaged and marketed to healthcare organizations to advance the cause of full integrated care in your communities. We are seeking a diverse group of participants for this collaborative to foster peer support and practical experience to augment the training/technical assistance that will be provided.  Applicants that have just begun to examine opportunities to contract with healthcare organizations are encouraged to apply.  We are also seeking intermediate level applicants-those that have begun exploring integrated network hub collaborations to expand their bandwidth to contract with healthcare organizations.  Applicants in the latter category should have already begin examining local partnership opportunities and should be prepared to identify potential network hub partners.
 
Applicants and their partners must be committed to the project and willing to provide appropriate time and resources from their organizations to actively participate in this learning collaborative (see “Participant Expectations” in the Network Development Learning Collaborative Charter).
 
Organizations that are NOT eligible to participate. Those who are participating in or who have been selected to participate in a concurrent business acumen learning collaborative with an overlapping time frame; Previous completers of the Network Development Learning Collaborative.
 
An in-person kick-off call will be held on May 21, 2018 from 1:00 pm to 5:00 pm during a preconference before the Center for Healthy Aging Annual Meeting in Arlington, VA. For those who are unable to attend the Annual Meeting, webinar capability will be available.

Ongoing monthly NDLC webinars will be held on the 3rd Tuesday of each month from 3:00 pm to 4:30pm ET from June 2018 through March 2019.
