Dear Project Infuse Participant:
This survey is designed to assess the use of engineering concepts to teach science. The purpose of this anonymous questionnaire is to determine how the program is working. It should take about 15 minutes to complete. Your responses will be anonymous, and the web-based format will not allow any responses to be linked to you.

To conduct the research and to maintain your anonymity, we have established a unique identification number for each participant that you will keep track of throughout the project. This identifier will consist of your birth date (mmdd) + the last four digits of your social security number. For example, for a person born on June 22, 1950 with a social security number of xxx-xx-0328, the INFUSE ID number would be 06220328. This will provide a way for us to have the information needed to conduct our research while maintaining the privacy of each participant.
