The PKD Foundation of Canada (PKDFOC) leads the national fight against polycystic kidney disease (PKD) through research programs, education, awareness and support to help discover vital treatments for PKD and improves the lives of all it affects.

The purpose of this survey is to provide the PKDFOC with information to help serve you better. If you are living with PKD or are a family member, caregiver or friend of someone living with PKD, then this survey is for you!
