Estate Planning Council of Northeast Wisconsin Lifetime Achievement Award Nomination Form

The Estate Planning Council of Northeast Wisconsin, Inc. is proud to recognize individuals whose careers have demonstrated exceptional dedication, professionalism, leadership, and service within the estate planning community.

Please use this form to submit your nomination for the Lifetime Achievement Award by October 30, 2026. Nominees should meet the following criteria:
1. Someone who has been in practice, in their profession for over 30 years and has been a member of the Estate Planning Council of Northeast Wisconsin Inc.;
2. Has exhibited long term excellence as a professional in one of the approved professions per membership; and,
3. Has affected his or her profession in the community in a unique and positive manner.
