A number of parents of children with disability have expressed extreme dissatisfaction with mental health care for their children in Western Australia.

This survey has been created by a group of parents to collect information about these issues. The intent is that the information will be presented to the Ministerial Advisory Council on Disability with a request that the issues are raised with the Hon Helen Morton MLC, Minister for Disability and Mental Health in WA.

The survey is anonymous and all information will remain confidential. However, if you would like your current issue directly raised with the Minister, there is a checkbox with the option to provide your name and callback details.
