2010 Edna A. Lauterbach Scholarship Fund Application |
1. Introduction
The fund was established by the Health Care Providers Self-Insurance Trust (HCPSIT), an affiliate of the New York State Association of Health Care Providers, Inc. (HCP) and is administered by The Community Foundation for the Capital Region.
Application deadline is June 1, 2010.
Candidate Eligibility Requirements:
1. Registered Nurse or Licensed Practical Nurse developing a home or community-based care business.
2. Reside in New York State.
3. Plan for or engaged in an entrepreneurial venture utilizing nursing or related home and community-based services.
4. Show that proposed class/training will improve candidate’s ability to manage their organization or venture.
5. Demonstrated financial need. Selection Process:
1. The Community Foundation shares applications with an HCP committee which reviews applications and selects recipients based on qualifications.
2. Awards will be announced at HCP’s Annual Meeting in the fall. Scholarship Fund Disbursement:
1. Number and size of scholarships vary based on applications submitted.
2. Recipient is required to submit a letter to The Community Foundation for the Capital Region at the end of the funded program describing the effect the class/training had on the recipient’s business.
3. Payments are made directly to the college or university in which recipient is enrolled. If recipient attends another type of institution (i.e., computer training company), recipient will be reimbursed the scholarship amount after submitting a paid receipt.
