SEPAC Survey Introduction

Dear Parents of Children with an IEP (Individualized Education Programs) or 504 Plan. We need your feedback to help shape the future of education for our kids.

The Lexington Special Education Parent Advisory Council (SEPAC), in collaboration with the Lexington Public School administration, is administering an anonymous parent survey to all parents with children in Special Education or with a 504 accommodation plan. 

A dedicated group of fellow parents, LexSEPAC advises the district on the education and welfare of our children in special education.  We are interested in your perspectives on placement and service delivery, school partnerships and communication. Our aim is to provide some meaningful feedback to Lexington Public Schools, to aid in the development of the Special Education and 504 process.

Although you will be asked to provide optional basic demographic information about your child, you will not be asked to provide any other personally identifying information. Demographic information will be used to better understand possible trends across groups.

  • Please take this survey once for each child in your family, on an IEP or 504 plan.
  • Please only one parent to submit a response per family.
  • Please respond to questions based only on the current year or most recent year accessing special education services.
  • We value your comments as much as we value your answers. You can add comments after each section and at the end of the survey.
  • This survey should take approximately 5-10 minutes to complete.
  • All responses, including comments, will be kept strictly anonymous and will be collated by the parent LexSEPAC group.
  • Results will be presented by LexSEPAC in the Spring of 2020.
