Mental Health Survey for Transitional Age Youth in Northern Santa Barbara County
Thank you for your interest in this survey! We want to hear from individuals 16-25 years old in northern Santa Barbara County. This anonymous survey allows you to share your thoughts, without identifying information attached to your responses. The goal is to better understand your experience of wellbeing, mental health services, and any unmet needs in the community. Survey questions will ask for write in responses, or will ask you to select one or more options from a list of choices. We will ask you to reflect on your own experiences. Individuals who complete the survey have the option to receive a $10 Amazon gift card.
To receive your $10 gift card, you must complete the entire survey. After you finish, you will be provided a link to a separate survey to collect your email address and contact information. You must provide an email address in this second survey in order to receive a gift card. After the survey is complete, you will receive a message from Rybbon with instructions to redeem your incentive.
Most of the questions in this survey ask directly about your mental health and wellbeing. You are welcome to skip any question without penalty.
