Social Housing COVID19 VAX Uptake Survey

Counterpoint Community Services has started a 'Covid Vax Assist' project, assisting social housing tenants in the City of Sydney Council local government area to access Covid vaccines by helping them to access and navigate the Covid Vaccine Booking system.

Data collected will provide helpful insight into the vaccination rates in our social housing communities.

All fully vaccinated tenants or those who we assisted in booking appointments and subsequently got vaccinated (undertaken from the advice of a health practitioner) will be eligible to be included in Counterpoint's monthly prize draws.

Your information will be treated with confidentiality in accordance with the law.
