An on-site flu-shot clinic will be held Tuesday, September 17, 2019 from 9:00am-2:00pm on the Campus Center Stage.
COSTCO pharmacy will be administering the vaccine. COSTCO will have at this clinic the same 4-strain flu vaccine for the same cash price of $19.99 that they offer at their pharmacy.
There is no charge at this clinic or at COSTCO Pharmacy if using your district health insurance card. Flu vaccination should be covered 100% on all Shasta College plans. COSTCO will also accept Medicare at this clinic.

*Costco is bringing vaccine indicated for adults; they are not bringing pediatric-dose vaccine.
* If you have questions or concerns about receiving a flu shot please contact your primary healthcare provider for guidance and direction.
*Employee Wellness Incentive: You must attach a copy of your flu shot receipt to the “STSIG Wellness Incentive Tracker” document found at http://www.stsigjpa.com/health-programs/wellness/ . Submit the incentive tracker to STSIG for credit.
*You do not have to pre-register for this clinic. A reminder will be circulated closer to September 17th.
If Tuesday September 17th is not convenient you may visit COSTCO Pharmacy when convenient. If you desire a variation of the vaccine (specific to children, pregnant women, etc.) please contact your healthcare provider.
