To ensure the quality of the program delivery, we ask the Leader who conducted the workshop and Monitor (third-person who monitored the workshop) to complete this form. Hawai‘i Healthy Aging Partnership requires Leader and Monitor to complete this form once for every workshop except the Leader who offers the workshop for the first time. A leader who offers the workshop for the first time, please complete the form after each session of the workshop.
