Cosplay

Statement of Confidentiality and Instructions
 
The following exploratory research is on the experiences/relationships/communities of people who cosplay. The goal is to understand how participating in cosplay and potentially cosplay communities has influenced cosplayers in their relationships and experiences during their lifetime.
 
Consent for Participation in the Survey Monkey Research

1. My participation in this project is voluntary. I understand that I will not be paid or receive any other benefit for my participation. I may withdraw and discontinue participation at any time without penalty. I am free to decline to participate or withdraw from the study with zero repercussions.
2. I am at least 12 years of age or older (for those under 18, permission needs to be given by a parent/guardian).
3. I understand that most interviewees will find the discussion interesting and thought-provoking. If, however, I feel uncomfortable in any way while taking the survey, I have the right to decline to answer any question or to end the survey.
4. Participation involves taking a survey through the URL provided by New Mexico State University through Survey Monkey. The survey will last approximately 15-20 minutes.
5. I understand that the researchers will not identify me by name in any reports, presentations or publications using information obtained from this survey, and that my confidentiality as a participant in this study will remain secure through password protection. All data will be deleted in October of 2021.
6. I understand that only the researchers will have access to this information. This precaution will keep my responses anonymous and therefore prevent my individual comments from having any negative repercussions.

8. For those who are part of the New Mexico State University system, each campus provides counseling for participants who may experience distress.
9. I have read and understand the explanation provided. By doing the survey, I am  providing my consent for this research.

Parental Permission for Child’s Participation in Research
I have read this form and the research study has been explained to me. I have been given the opportunity to ask questions and my questions have been answered.
