* 1. My family is treated with respect and dignity at JMS.

When my child has a problem at school it is addressed quickly and resolved by the appropriate staff member.

Problems are resolved, but it takes a few phone calls/emails to get it addressed.

Problems never get resolved and I end up really frustrated.

I don't have an opinion either way, this question does not apply to me.

* 2. My child has the necessary classroom supplies and equipment for effective learning.

JMS is delivering challenging core academic lessons and has the equipment to do it.

JMS is delivering challenging core academics, but they could use some updated materials/ textbooks.

My student is not very challenged at JMS and they could increase the rigor a little.

JMS is lacking in both challenging core academics and equipment.

* 3. I regularly receive feedback/communication from school staff on how well my child is learning.

JMS engages families in meaningful ways to keep us informed of my child's learning process.

JMS provides lots of communication, but I wish more of my child's individual teachers would contact me.

JMS does provide lots of information, but i with they used a different social media platform.

JMS sends out way too much information.

JMS does not send enough information, I never know what is happening.
