The Healthy Smiles for Mom and Baby program is funded by a four year, Health Resources and Services Administration grant received in August, 2015. Children’s Health Alliance of Wisconsin is leading these efforts with guidance from a multidisciplinary project advisory board which includes representation from the Wisconsin Dental Association.
We are gathering information from dental providers in Wisconsin to better understand current practices and opinions on oral health care for pregnant women. As a representative of the dental provider community, you are one of the most important components of this project. We need your input to gain a better understanding of your experience managing pregnant patients in your practice and during your dental education.
We are requesting your help in completing a brief survey that should take approximately 5 minutes of your time. Please submit your responses before September 9, 2016. Participating in this survey is voluntary and anonymous.
