Introduction

Every year people from health and the local council collect information about the needs of the people who live in the area.

This information is used to plan for the services that people need.

The Mental Health and Learning Disabilities Partnership Board in York has asked Inclusion North to collect information about what is important to people with a learning disability and their family member/carers living in York.

The questions we are asking will mean different things to different people depending on your experiences so we want you to answer the ones that are most important to you. Your answers don't have to be long.

We will collect all of the information from this survey and from the engagement sessions we are running. The information will be given to the Partnership Board so that they know what is important for people in York.
