Thank you for taking the time to nominate a teacher, volunteer or staff member for his or her positive impact on our students, our schools, and our community. Minnetonka Schools is proud to announce its 13th Annual Celebration of Excellence Recognition Program, honoring adults who positively impact our students and our educational community.

All awards will be presented at a Gala Celebration of Excellence at the Arts Center on Thursday, May 10, 2018.

NOTE: There are two pages to this nomination form. Before completing the survey, please think about what you want to say about the nominee: How does the nominee demonstrate child-centered excellence? How is the nominee innovative? Is there a story you can share about a time the nominee made a difference in the life of a child? What are the strengths or unique qualities of the nominee?

HINT: Personal stories about how the nominee impacted the life of a student or work of colleagues receive the most attention by the selection committee.
