Eye Believe Survivorship Seminar – Lodging Assistance Request
A Cure In Sight (ACIS) is pleased to offer limited lodging assistance for attendees of the Eye Believe Survivorship Seminar, made possible through the generous support of Eye On Grace.
This assistance is for hotel accommodations only. Approved lodging expenses will be paid directly to the designated hotel by ACIS.
Please note the following before completing this request:
Lodging assistance is available for approved hotel room charges only. Travel expenses, including airfare, mileage, rental cars, rideshare, parking, meals, and other transportation-related expenses, are not included or eligible for reimbursement at this time.
Guests are responsible for any personal or incidental hotel charges, including room service, parking, upgrades, additional nights, or other expenses not specifically approved by ACIS.
If you receive lodging assistance and are unable to attend the Eye Believe Survivorship Seminar, you are responsible for notifying the hotel and ACIS as soon as possible. Any cancellation, late-cancellation, or no-show charges resulting from your reservation will become your responsibility.
Completion of this survey does not guarantee lodging assistance or confirm a hotel reservation. Assistance is subject to eligibility, availability, and available funding.
Once your lodging request has been reviewed, ACIS will provide additional information regarding approval and hotel reservation details.
