Sunbury Health Centre uses a variety of ways to monitor performance and improve services using patient feedback.
This survey is being carried out by the Patient Participation Group (PPG); a small group of Sunbury Health Centre patients who act in a voluntary capacity to represent the patient population.

Significant changes in the ways in which the Practice offers its services to patients have taken place more recently. We are therefore inviting feedback from patients who have interacted with the Practice during the
past 18 months.
This is an anonymous survey which should take no longer than 10 minutes minutes to complete. Responses will be analysed by members of the PPG.
This survey not only aims to collect feedback that will be useful to the Practice, but also seeks to communicate some of the key changes that have taken place within the Practice or the wider NHS since the last PPG survey in 2023.
Changes include:
- Self-booking appointment links offering a choice of clinicians, dates and times, except for patients who need to be seen urgently ie. same day (increasingly offered since Summer 2023)
- Different Practice website: launched Autumn 2023
- Pharmacy First service: launched January 2024
- Telephone call back service: launched April 2025
- GP led assessment of all medical requests (triage): launched June 2025
- Development of the NHS App: the preferred route for patient information and requests as opposed to using Patient Access
