We are seeking members for the Ontario Health West Patient and Family Advisory Council.
We want to work with you so that we better understand your health care experiences, needs, and priorities. You can play an important role in shaping how our province designs and delivers health care that meets the needs of your community and improves people’s healthcare experiences.
We need your help to do this work!

You can advise us on topics that align with Ontario Health’s provincial and regional priorities such as mental health and addictions, health equity, primary and community care, digital transformation and Ontario Health Teams (OHTs).

- People who regularly use or support people who use the health system. For example: seeing your family doctor, receiving care for mental health or addictions, or being a patient or caregiver for someone in the hospital—in the last two years.
- People who have some understanding of Ontario’s health-care system, and how the parts of the system connect to each other.
- People who are interested in improving care for others in their community.
- People who may be underserved by our current system. This will help ensure our health services meet the diverse needs and priorities of all Ontarians.
What do we need from you?
- A two-year commitment.
- Attendance at meetings (virtually) a minimum of six times per year. (Support will be provided to ensure you are able to attend meetings and participate fully).
- An interest in collaborating with other patient advisors across the health system and participating in ongoing training and educational opportunities.

By completing the Expression of Interest Form indicating your interest in participating in the Ontario Health Central Patient and Family Advisory Committee, you have consented to having Ontario Health collect and store your personal information for the purposes of committee membership selection. Ontario Health will collect your personal information including your name, demographic and contact information, and relevant experience. All information you share will be kept confidential and your privacy will be protected except as required by law. Your information will be retained until the selection of committee members has been completed. With your approval, your information may be retained for future opportunities. If you are selected to participate on the committee, your information will be retained for the duration of your term which is 2 years. Information will be securely destroyed as per Ontario Health policies.
