The San Francisco Marin Medical Society (SFMMS) Community Service Foundation (CSF) was founded
decades ago with two primary purposes:  (1) To provide a tax-exempt 501©3 entity for SFMMS members and others to receive grants for medical and public health-related projects, with an administrative overhead rate much lower than most university or other larger routes; and (2) to disburse funds to worthy local medical/public health related efforts.

The CSF board is the SFMMS Executive Committee, which makes decisions regarding these functions.
Recent grants have been made to the San Francisco Free Clinic, Operation Access, and the Marin Canal Alliance.

We are asking SFMMS members to nominate worthy new potential grantees using this form.
In 200 words or less, please describe how the organization you are nominating meets the following criteria:

  • Operates in San Francisco and/or Marin Counties.
  • Contributes to the provision of medical care and enhances public health, especially among underserved patient populations.
