Dear Friend,

“Aging adults” is an important topic of discussion to everyone – especially to us. Your thoughts and opinions will help us plan for the future so we can meet the needs of older adults and their families – both now and for generations to come.

The Jewish Association on Aging is a continuum of care offering home health care and residential services. Our mission is to honor and enhance the lives of older adults through a spectrum of individualized, quality care consistent with Jewish values and traditions. Currently, our network includes assisted living, memory care, skilled nursing, home care and hospice, adult day services, outpatient and short-term rehabilitation, care navigation and a kosher home-delivered meal program.

Our organization is undergoing comprehensive strategic planning efforts to identify and plan for the diverse needs of our community members for years to come. Before we move forward, we would like to learn more about what our friends, supporters and the local community currently know and think about the JAA.

To show our appreciation for your participation, we are offering a chance to win one of five $100 VISA® Gift Cards. To enter, simply complete this survey by Monday, December 15 and be sure to include your contact information at the end of the survey.

If you have any questions, please contact Beverly Brinn, Director of Development, at 412-521-1975. Thank you in advance for participating in this survey.

Sincerely,

Deborah Winn-Horvitz
President & CEO
