This explains why we're asking these questions and how the information is used.

We would be grateful if you could complete this equality monitoring form.  It is kept separate from your application and the information you give will be kept confidential from the selection panel, following Data Protection requirements, and will only be used to monitor and analyse the progress of the Theatre’s Equality, Diversity and Inclusion Action Plan, in a way that maintains anonymity.

This monitoring form is compiled following guidance from Arts Council England’s data monitoring and allows us to report as accurately as possible.
