Assessment to Action Tools - Interview Eligibility Questionnaire

Health Resources in Action, in partnership with National Network of Public Health Institutes and CDC, are evaluating the Sodium Reduction: Assessment to Action Tools and corresponding website (www.assessmenttoaction.org). We are interested in hearing from users of the tools about their experience completing the tools and any feedback they have. To be eligible, participants must operate/manage a food service setting (cafeteria-style, congregate meals, vending machines, concessions/grab-n-go), or be able to partner with a food service manager to complete the Assessment to Action Tools. Please complete this short screening questionnaire to determine eligibility for participating in the evaluation. For completing an interview, you will be entered into a drawing for a $25 gift card (multiple winners)!
