Innovation Intake Form

Thank you for your interest in participating in the Family Medicine Innovation Forum’s Call for Innovations. The FMIF is an opportunity for you to share your innovation with a wider audience.
The ACFP is committed to provide a platform for collaboration, discussion, and experimentation that will offer members, primary health care teams, and health partners opportunities to spread and scale latest innovations for Alberta’s primary health care.

We hope that the FMIF will:
  • Showcase and recognize innovations and best practices that improve care and access for Albertans to family medicine and primary health care, decrease administrative burden, and advance team-based primary care
  • Demonstrate the power of data within the health care system for system improvements, workforce planning, predictive modelling, artificial intelligence, population health management, interoperability, and resource allocation
  • Facilitate connections between innovative health providers and programs with community providers, citizens, and/or partners, with the means to assist with the spread and scale of their program(s)/initiative(s)
How It Works:

The FMIF Scientific Planning Committee will review all submissions and shortlist finalists for an online vote. Innovation finalists will be asked to submit a short video ‘elevator pitch’, which will be featured on an ACFP site during the voting period. Results will help determine which top projects will be invited to present on stage for the forum.

Deadline for submission is Monday, June 15, 2026.
