COVID-19 Survey to Retina International Membership

Introduction and Consent

Purpose of the Survey:

Retina International is inviting you, our membership organizations, to take part in an anonymous survey to assess the impact the COVID-19 pandemic and associated restrictions has had on your organization.

The survey contains 11 questions and will take approximately 4 minutes to complete.



Confidentiality

The information collected as part of this survey will be kept strictly confidential and the identity of your organization will not be revealed. Information will only be accessed by authorised personnel where required for the purpose of administering the survey and conducting the analysis. Individual data will be permanently deleted upon completion of this survey.
