Making Services Better for Children and Young People - Parent Carer Survey

Why are we doing this survey?

Last year we heard from children and young people across Cheshire and Merseyside. They told us what helps to make appointments feel right for them, things like;
  • being spoken to clearly
  • understanding what’s happening
  • having a say in their care.
Some children and young people also shared when appointments didn’t feel so good, like;
  • times when they felt invisible
  • time when they felt left out of their appointment.
As a result of this feedback, Beyond has teamed up with the ISupport team to try and help make using health and social care services better. Now we need your help!

We’d love to know which services you think ISupport should visit first!
Your voice matters, and we really appreciate you sharing your views with us!

This form isn’t a way to make a complaint. If you or your child are not happy with the care you have received then there are places which can support you like PALS (the Patient Advice and Liaison Service), who are there to listen and help.
This form is anonymous please do not add your name or any contact details.
First tell us a little bit about your child
1.How old is your child?
2.Do you live in Cheshire or Merseyside?
3.Does your child have any disabilities, neurodivergence or additional support needs?
4.Which of the following hospital services have you used with your child? (tick as many as you like)
5.Which of the following community services have you used?
6.Which of these services could be improved and why? (tell us about as many as you like)
7.Which of these services were good and why? (tell us about as many as you like)
Thank you so much for sharing your views, we will use this information to focus our work over the next year.
