2018-2019 Student Application for the Inteprofessional Student Hotspotting Learning Collaborative |
Student Hotspotting is an educational curriculum originally developed by the Camden Coalition of Healthcare Providers. This program facilitates teams of interprofessional students learning about the challenges faced by complex patients during their interactions with the current healthcare system. Students from different health professions work together over six months (September-March) to learn how to identify "super-utilizers" and deepen their understanding of the factors that lead to high healthcare utilization and multiple readmissions. Students assist "super-utilizing" patients with care coordination and health system navigation. They also learn how to advocate for patients’ needs with providers and local social services.
Upon successful completion of the program, students may be eligible to receive transcript designation of Excellence in Collaborative Practice, depending upon their College’s/Program’s specific stipulations.
